The RN Case Manager is responsible for assessing and managing a patient's care under the direction of the attending physician and the medical director. A RN Case Manager is assigned to each patient to foster a trusting relationship and for the continuity of care. All case managers at Chattahoochee Hospice are registered nurses.
The nurse's license authorizes her/him to perform professional assessments of patients, create nursing plans of care, perform many skilled nursing procedures, and provide all the necessary aspects of nursing bedside care. The RN Case Manager is authorized to supervise, direct and educate other nursing staff.
RN Case Managers are required to maintain their professional competence by completing annual continuing education training, follow the Code of Ethics for all nurses and provide care which meets the Standards of Care.
RN Case Managers are intermediaries in communicating the patient's status and needs to the physician. Because of the nature of hospice, the nurse's role is expanded and becomes even more important to your loved one. The RN Case Manager communicates directly with the physician notifying him/her what symptoms have arisen and generally providing all the information necessary for the physician to write medical orders for the patient.
It is important for you to notify the RN Case Manager about any needs or problems your loved one may be experiencing.
