We pledge to provide quality programs, services and technology to meet the unique physical, emotional and spiritual needs of the terminally ill and their families, and to educate the community about end of life issues.

Vision:
Provide access to the highest quality hospice care for our community with a comprehensive approach to pain and symptom management and compassionate customer service while developing exceptional professional skills regarding hospice care for our community.

Goals:

• To provide service excellence in nursing care, emotional support, spiritual guidance and bereavement services to terminally ill patients and their families/caregivers.
• To provide a spirit of acceptance and mutual respect, quality and compassionate care to all patients.
• To promote independence, comfort, and dignity surrounding death in one's chosen environment.
• To serve as a facilitator of education and community resources for patients with limited life expectancy.

Control - You are in control of your care. Chattahoochee Hospice will work with you, your family and your physician to ensure you get the care you need.

Comfort - Your comfort is our primary concern. Freedom from physical pain is our first goal.
