Hospice social workers are concerned with enhancing quality of life and promoting well-being for patients, caregivers and family members. Hospice and palliative care social workers fulfill a variety of roles and tasks and spend much of their time directly with patients in their homes, inpatient settings, or office consultations.

- Assessment of the goals, needs, and strengths of both the patient, caregiver, and family members
- Emotional counseling for patients, caregivers, and family members
- Providing psycho-social education to patients and family caregivers about coping skills, hospice and palliative care philosophy, and non-pharmacological symptom management strategies
- Planning for admission, discharge, and coordinating care
- Facilitating advance care planning and lifespan planning to include providing information about important legal documents and services
- Identifying and linking patients with community resources and agencies and their services and qualifications
