Hospice social workers are concerned with enhancing quality of life and promoting well-being for patients, caregivers and family members. Hospice and palliative care social workers fulfill a variety of roles and tasks and spend much of their time directly with patients in their homes, inpatient settings, or office consultations. 

Our licensed social workers are your advocate and provide the following services:

• Assessment of the goals, needs, and strengths of both the patient, caregiver, and family members
• Emotional counseling for patients, caregivers, and family members
• Providing psycho-social education to patients and family caregivers about coping skills, hospice and palliative care philosophy, and non-pharmacological symptom management strategies
• Planning for admission, discharge, and coordinating care 
• Facilitating advance care planning and lifespan planning to include providing information about important legal documents and services
• Identifying and linking patients with community resources and agencies and their services and qualifications
